Print the document ensuring your printer is set to 'landscape' and 'A4' paper size.

Each document has 2 player registration forms.

Once completed and signed the forms can be scanned and emailed to Mrs Toni Muller at csfl.registrations@yahoo.co.uk or posted to

PLAYERS CAN BE SIGNED THIS SUNDAY 16TH SEPTEMBER AND ALL PLAYERS WHO PLAY THIS SUNDAY MUST COMPLETE THE FORM. JIMMY WILL BE AT DUPPAS HILL AND PURLEY WAY WITH FORMS IF YOU NEED THEM. PLAYERS MUST BE SIGNED UP PRIOR TO PLAYING ON 23RD SEPTEMBER 2007.
